How to request your personal information

Accessing your information

You have rights to access your own information under the:

Medicare and Pharmaceutical Benefits Scheme (PBS) claims information

You can get your Medicare and PBS information by sending a request to:

You can request information by:

  • completing one or more of the following forms, as required
  • writing to Medicare Australia, providing the same information as required on the relevant form

To request Medicare or PBS information for an individual or a family:

Note: all family members aged 14 years and over must sign the application form.

To authorise Medicare Australia to release personal Medicare or PBS information to a third party, for example an insurance company:

Note: Medicare and PBS information is more readily available for the past five years.

Requests for information under the Freedom of Information Act 1982

You can request access to information held by Medicare Australia under the Freedom of Information Act 1982 (FOI Act). The FOI Act gives members of the public certain rights to access official documents held by Commonwealth Government agencies.

Note: requests for your own personal Medicare and/or PBS claims information can be obtained outside of the FOI Act.
